    Job Description



    Directly accountable to the Director of Nursing Services (DNS) to manage the nursing care of assigned residents to maximize independence and self-direction for residents. Responsible for safety of residents under their supervision. Manages day-to-day compliance with Federal and State regulations, Center policies and procedures, and standard of practice with regard to resident care. Validates nursing staff functions within the scope of their practice and complies with regulations, policies, and procedures. Performs other duties as assigned by supervisor.


    Resident Care

    1. Interprets existing policies and procedures to other nursing personnel and nursing assistants, restorative nursing assistants, residents, families, and physicians.

    2. Assesses resident needs and initiates development of individualized resident care plans. Utilizing the nursing process, Minimum Data Set (MDS), Care Area Assessment (CAA), and care plan:

    a. Reviews and updates resident care plans and Certified Nursing Assistant (CNA) flow sheets as necessary and as assigned in compliance with regulations.

    b. Conducts interdisciplinary resident care conferences as scheduled.

    3. Supervises and evaluates direct resident care provided within assigned unit and initiates corrective action as necessary.

    a. Evaluates resident care as related to individualize resident needs, family involvement, and the physician’s plan of care for the resident.

    b. Obtains report from nurse and records sufficient information to implement appropriate follow-up action in relation to the residents treatment needs.

    c. Provides nurse on duty with concise, accurate report and takes follow-up action as necessary.

    d. Identifies special nursing problems and emergency situations quickly, and initiates appropriate interventions for individual residents.

    e. Administers treatments per physician order, is responsible to document per Center procedures.

    f. Supervises and evaluates the implementation of Resident Bill of Rights.

    g. Documents resident care provided and resident’s response or lack of response to treatment provided.

    h. Assesses and reports changes in resident’s condition, including development of pressure ulcers, to physician, the DNS and responsible party, and takes recommendations for nursing action to be implemented.

    i. Assesses resident responses to medication and treatment, and makes appropriate recommendations for nursing action to be implemented.

    4. Assesses on a weekly basis via resident rounds the condition of existing pressure ulcer by stage, size (measurements), sites depth, color, drainage, and odor. Reports problems to the DNS; takes necessary follow-up action.

    5. Assists in the admission, transfer, and discharge of residents as necessary and assigned by DNS. Completes admission, transfer, and discharge documentation.

    6. Renders immediate assistance to residents in need.

    7. Supervises the timely response of call-lights within assigned unit.


    8. Listens to resident, family, or physician complaints and initiates corrective action and/or reports special problems to the DNS.

    9. Follows up on physician orders, lab, and x-rays.

    10. Observes resident, records significant conditions and reactions, and notifies family (responsible party) and physician of resident’s condition, and reaction to drugs, treatment, and significant incidents. Documents it according to Center policy and Standards of Practices.

    11. Schedules resident appointments and notifies transportation for transport arrangements.

    12. Rounds with physicians as needed and process physician orders.


    1. Follows established performance standards and implements Center policies and procedures on the unit assigned.

    2. Participates in Center educational programs, such as orientation, in-service, and nursing assistant education programs.

    3. Provides clinical supervision to nursing assistants.

    4. Attends staff meetings and in-service classes regularly.

    5. Instructs nursing assistants in proper, preventive safety measures and use of adaptive equipment to meet the needs of each resident in coordination with other nurses and Staff Development.

    6. Assists in control of supplies and equipment within the unit.

    7. Identifies performance concerns with supervised staff, counsels them about performance, and invokes disciplinary action if necessary. Complete employee evaluations.

    8. Compiles information for Center committees as instructed, such as Weekly Skin Report and Weekly Matrix/Resident Roster.

    9. Observes infection control procedures performed by staff to validate compliance.

    10. Works cooperatively with residents, nursing personnel, physicians, families, consultant personnel, community agencies, and ancillary personnel. Follows up on residents and/or family concerns.


    1. Observes Center safety policies and procedures.

    2. Consistently presents an appropriate, professional appearance per Center dress code.

    3. Comes to work as scheduled and consistently demonstrates dependability, punctuality, and a professional demeanor.

    4. Assumes accountability for knowing and complying with policies and procedures contained in the employee handbook.

    5. Accepts assignments in a mature, cooperative manner.

    6. Stays current in theory and practices through continuing education and reading.


    1. Other duties as assigned and appropriate to the position.


    1. Licensed as a Registered Nurse or Licensed Practical Nurse.

    2. One-year previous supervisory experience in hospital, skilled nursing center, or other related medical facility preferred.

    3. Training in rehabilitative and restorative nursing practices preferred.


    4. Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.

    5. Requires mental maturity to deal with fast-paced work environment, with occasionally combative residents.

    6. Tolerance for body secretions, strong odors, exposure to diseases, and varied room temperatures.

    7. Demonstrated ability to communicate in English (reading, writing, speaking, and comprehension) in order to understand, plan, and carry out resident care plans and treatments.


    1. Reports to the Director of Nursing Services.
