    Job Description
    Full Time Non-Exempt Shared Living Coordinator 
    Shared living is an arrangement in which care is provided to an individual with a disability in a private home.  Shared living is built on the foundation of life sharing, developing natural supports, and being an active member of the community. The shared living provider/companion provides services, including assistance, support, and guidance, in life domain areas such as daily living, safety and security, community living, healthy lifestyle, spirituality, citizenship, and advocacy.  

    The Shared Living Coordinator is responsible for coordinating services for individuals with disabilities in a shared living, community living environment.   The Shared Living Coordinator will maintain ongoing positive communication with all stakeholders, actively participate in the person centered planning process for participants, and ensure person centered plans are being implemented as written.  An Associate’s degree in human services, social work, psychology, related field or an equivalent combination of education and experience is desired. The Shared Living Coordinator must have the presence, stature, and personality to effectively represent LifeQuest to the public and other professionals. The hours are flexible and may include some evenings and weekends.  Some out of town travel using a personal vehicle may also be required.  
    Wage dependent on education and experience.
